Keeping Patients Connected: Innovative Models for Sustainable System Growth
As health systems seek to expand access, reduce patient leakage and strengthen connections with their communities, rethinking primary care delivery has never been more critical, but building the "right" primary care model is complex. Building the ideal primary care model requires strategic alignment, operational foresight and a clear understanding of the care delivery sphere.
In collaboration with the Association of Departments of Family Medicine (ADFM), this session brings together academic and clinical leaders to explore innovative, real-world approaches to primary care access, ranging from in-house expansion to joint ventures and partnerships with community medical groups, including Federally Qualified Health Centers (FQHCs). Through case-based insights, participants will learn how different systems are designing and delivering primary care access models that are financially sustainable, operationally scalable and tailored to their organizational goals.
Whether an organization is building from within or evaluating external partnerships, this session will help better define a strategy, avoid common pitfalls and unlock the full value of primary care in today’s healthcare environment.
Learning Objectives:
- Compare multiple models for expanding primary care access including in-house practices, joint ventures and partnerships with community medical groups including FQHCs.
- Analyze care delivery strategies that reduce patient leakage, improve access and enhance continuity within integrated systems.
- Identify operational and financial considerations that impact the sustainability and scalability of different primary care models.
